It's fine during the warm season, but I'm worried about skin redness whenever winter comes.

It used to be fine, but for the past few years, whenever the weather suddenly turned cold and I go out, I get hives on my face.
Redness is an added bonus, too. Even though I make sure to apply moisturizer diligently, it doesn't seem to have any effect.
When I go out briefly in front of my house, I make sure to wear a mask when it is cold.
What could be the problem? Is it due to a weakened skin barrier? How should I manage my redness?
