How to remove old blackheads

I want to remove blackheads that are decades old. Is there no way to do this other than visiting a dermatologist? Is there no method to remove them through continuous and repetitive treatment?

It grew next to my nose and hasn't gone away; it just keeps staying, and I'm really worried about it. Please, someone help me.
