Are there any pads that remove freckles and blemishes?

I've had so many freckles and blemishes since I was young that I live with concealer...

I even went to a dermatologist and got treatments for melasma and blemishes, but the effects were only temporary.

I want to get rid of freckles and blemishes with home care rather than treatment. Do you have any products that you have seen results with?
